About COVID-19 Vaccination Booster(Third Dose)

This is the latest information about COVID-19 vaccination booster. It is subject to change.

Eligibility

Those 18 years and older who have a resident record in Onomichi City are eligible to receive the vaccine. Also, you can receive the vaccination booster if more than 6 months have passed since the second vaccination.

Vaccination Vouchers

The vaccination vouchers will be sent to eligible people when your vaccination period approaches. The color of the envelope Onomichi City will send is purple. Please make sure to open it and check the contents.

How to make an appointment

Vaccinations are conducted in two ways: individual vaccinations (held at medical institutions) and group vaccinations (held at venues which Onomichi City prepares).

You need to make an appointment in advance. You will need your vaccination voucher number when you make your appointment. Please make an appointment after you receive the voucher.

Individual Vaccinations

Please contact the medical institution at which you would like to be vaccinated directly.

Please make an appointment by phone or online after you receive a vaccination voucher.

What to bring to the vaccination site

・Vaccination voucher

・Completed Prevaccination Screening Questionnaire

・Personal identification (passport, residence card, etc.)

・Health insurance card

・Prescription Record Notebook noting any prescription medication you are currently taking  (those who have one)

・Maternal and Child Health Handbook (those who have one)

※Please wear clothes which easily expose the upper arm.
※Please wear a mask.

Cancellation

If you have a fever or feel sick on the vaccination day, please cancel your appointment and don't visit the vaccination site. In such cases, please call the following number to cancel your appointment. Also, if you have another appointment and can't go, please call the following number in advance.

Vaccination Cost

The COVID-19 vaccine is free.

Note: there have been cases of scammers pretending to be municipal or health center workers and asking for money for vaccinations. We will never ask you for money for the COVID-19 vaccine or to make an appointment. If you receive any suspicious phone calls, hang up immediately and report it to the police.

Relief System for Injury to Health with Vaccination

Vaccinations can cause health problems (illness or disability). Although this is extremely rare, the risk cannot be eliminated, and a relief system has been established for this reason.
In the case of health damage as a result of the vaccine from the COVID-19 vaccine, relief (medical expenses, disability pension benefits, etc.) is also available under the Immunization Act. Please consult with the municipality where your residence is located regarding the procedures required for application.
